Social prescribing allows people of all ages to take greater control of their lives and health. It addresses people’s needs in a holistic way, recognising that health is determined by a wide range of social, economic and environmental factors.

GPs and primary care teams can refer patients into non-medical support and activities to support or benefit their health and wellbeing.
