The Community Navigation service is a community resource that aims to improve people’s quality of life, health and wellbeing by recognising that this can be affected by a range of social, economic and environmental factors.
Covering East Kent, the service is available to all residents aged over 55 years, regardless of whether they are receiving any other services from Adult Social Care.
It is also available to those under the age of 55 years who have complex needs.

Based in the community and delivering a social prescribing service from community venues, we support people before they reach a crisis point, and address specific needs.
Working alongside a community supermarket enables our social prescriber to offer additional financial advice and support that people might need.
In Dover we are supporting people living with dementia and their families, and across East Kent we have a Macmillan Cancer professional delivering social prescribing advice and support to those with a cancer diagnosis.
